Lichen planus and lichen sclerosus

Lichen planus and lichen sclerosus are two different inflammatory conditions that share part of their name.

Lichen planus causes itchy, flat-topped, purple or dark bumps, often on the wrists, ankles and lower back. It can also affect the mouth, nails, scalp and genitals.

Lichen sclerosus causes thin, white, wrinkled patches, most often in the genital and anal area. It can cause itching, pain and scarring, and needs ongoing treatment.

Neither condition is contagious.

Signs and symptoms

• Itchy, flat, shiny bumps on the skin
• White lacy patches or sores inside the mouth
• Thin, white patches in the genital area
• Itching, pain, tearing or discomfort with intercourse
• Ridged or thinning nails, or patchy hair loss

How we treat it

• Strong prescription steroid creams and ointments, the main treatment for lichen sclerosus
• Other prescription creams and gels
• Oral medicines for widespread lichen planus
• Long-term follow-up, because genital lichen sclerosus increases the risk of squamous cell carcinoma. Persistent sores, thickening or new growths should be checked.
