Alopecia areata, traction alopecia and CCCA

These three conditions cause different kinds of hair loss, and each needs its own treatment. Early diagnosis matters, especially for types that can scar.

Alopecia areata is an autoimmune condition that causes round patches of hair loss on the scalp, beard, eyebrows or body. Some people lose all their hair.

Traction alopecia is hair loss from tension on the hair over time, from tight braids, ponytails, extensions, weaves or buns. It is often along the hairline.

Central centrifugal cicatricial alopecia (CCCA) is a scarring hair loss that starts at the crown and spreads outward. It is most common in women of African descent.

Signs and symptoms

• Smooth, round bald patches (alopecia areata)
• Thinning along the hairline or where hair is pulled (traction alopecia)
• Thinning at the crown, sometimes with itching, tenderness or burning (CCCA)

How we treat it

• Scalp exam and sometimes a biopsy to confirm the diagnosis
• Steroid injections into patches or inflamed areas
• Prescription creams, foams and solutions
• Oral JAK inhibitors for selected patients with severe alopecia areata, after reviewing eligibility, serious risks and monitoring requirements
• Hair care guidance to reduce tension and protect remaining hair
• Minoxidil and other treatments to support regrowth
