2. Dermatologists Bill by Complexity, Not Time

In dermatology (and most medical specialties), office visits are billed based on medical decision-making (MDM) —the complexity of the evaluation, diagnosis, and management—not the duration of the visit.

That means your bill reflects:

The number and seriousness of the conditions addressed
The data reviewed or tests ordered
The risk of complications or management decisions

It does not depend on how long you were in the exam room.

3. Why CMS Includes “Typical Time”

CMS includes a “typical time” with each code to help describe the average amount of time a provider might spend on that type of visit in total , including:

Reviewing your chart before the visit
Documenting your note afterward
Ordering labs or prescriptions
Coordinating care or referrals

So even if your face-to-face time was short, your provider may have spent significant time behind the scenes managing your care.

4. Example

Let’s say your dermatologist sees you for a changing mole.

They take your history, review prior biopsy results, perform a detailed skin exam, document findings, and decide that a biopsy is needed.

Even if that visit took only 15–20 minutes in person, the complexity of the medical decision supports a code like 99214 , which CMS labels as a “30-39 minute” visit.

5. What to Remember

The “minutes” listed on your bill are not a timer or clock of your visit .
Your dermatologist bills based on medical complexity , as required by national coding rules.
Every practice in the U.S. uses the same CPT codes and definitions set by the AMA and CMS.

I Paid My Copay—Why Am I Getting a Bill?

Clearing up "Copay Confusion."

Many patients are surprised to receive a bill after their visit when they already paid their copay, or didn't have a copay at all. It’s a common point of confusion and an important one to clear up.

Here’s why this happens and what it means for you.

What Is a Copay?

Some plans have a copay, others do not. A copay is a fixed amount you pay upfront when you visit a healthcare provider. It’s set by your insurance plan and doesn't necessarily mean it's the only out of pocket cost the patient will be responsible for. It's the minimum amount you might owe as the patient, but it's usually not the full amount you will owe.

Visits are billed to your insurance after completion, and depending on what was done during the visit and what your plan coverage is, you may owe nothing additional, or more commonly you will owe a portion of your visit to the practice, even when you have active, in network coverage. The amount you owe is called "patient responsibility" and can be made up of one or more of the following: copay, coinsurance, or deductible.

Why Am I Getting a Bill Now?

After your appointment, the provider sends a claim to your insurance company, which includes all services performed. Your insurance then reviews the claim and applies your plan’s rules. This process can take just a few days, or in some cases 30-90 days or more.

This process determines:

What portion of the visit they will cover
What amount goes toward your deductible (if you have any)
Whether coinsurance applies
Whether any part of the visit is not covered by your plan

Receiving a bill does not necessarily mean that services were not covered or your plan denied any part of the visit. You can still owe money under one of the above categories for a fully covered visit. In fact, most patients who visit a specialist practice (like a dermatology practice) will receive a bill.

Once the insurance company finalizes this, they notify both you and the provider. If any part of the cost is still your responsibility, a bill is issued for that remaining balance.

So while you may have paid nothing at your appointment, or you may have paid a copay, this often does not cover the full cost, even for a "fully covered" visit. That is why a bill may follow.

At My Visit, I was Told There Was No Charge

At your appointment, it's possible that there was no charge, which simply means that you do not have any copay (the portion that some plans require patients to pay at the time of service). While this may have been the case, it does not mean there is no charge to be seen by a provider.

After a visit is sent to your insurance plan for processing, depending on how your insurance plan is structured, you may be responsible for a portion of the visit. Once the visit is fully processed, your insurance plan sends us something called and ERA (similar to an EOB that you receive as the patient). This tells us how much, if anything, we must collect from the patient.

The money we are required to bill our patients can be due to a copay, coinsurance, deductible, denied services, or a combination of these.

Why Can't the Balance Be Discounted After Insurance Processes It?

This is another common question: "If I owe money after my insurance processes the claim, can't the provider just reduce or waive the balance?"

The answer is usually no, and here is why.

Healthcare providers sign contracts with insurance companies that legally require them to bill and collect the patient’s share exactly as determined by the insurance plan. These rules are put in place to keep billing fair and consistent for all patients.

If a provider reduces or waives your balance after insurance has processed the claim, it may be considered a violation of the contract with the insurer. In some cases, it could even be considered fraud. Exceptions are only allowed under formal financial hardship policies, which require documentation and approval.

What If I Still Have Questions?

If you're unsure about a bill or charge, here are the best steps to take:

Review your Explanation of Benefits (EOB) from your insurance. This document outlines what was billed, what was covered, and what you are responsible for paying.
Contact your insurance company to better understand how the charges were applied based on your plan.
Reach out to the provider's billing department to get a clear breakdown of your visit and the balance due.

Medical billing can be confusing, but both the provider and your insurance company should be able to help clarify what happened and why.

Why Your Dermatology Visit Isn’t Billed as Preventive Care

Two of the most common questions we hear are:

"Why did I get a bill? All I had done was my annual skin check, which my insurance covers at 100% with no patient responsibility."
“Why isn’t my dermatology visit considered preventive care like my annual physical with my primary care doctor?”

They are great questions. Here’s what you need to know about why dermatologists can’t bill preventive visit codes, and what happens if we try.

Preventive Care Is for Primary Care

Insurance companies—including Medicare—have strict guidelines on what qualifies as preventive care. These visits, such as annual physicals and wellness exams, are designed to be performed by primary care providers (PCPs), such as family medicine or internal medicine doctors.

These visits are billed using specific preventive care codes that are often covered in full by insurance plans.

Dermatologists, on the other hand, are specialists . Our job is to diagnose and treat conditions related to your skin, hair, and nails. Because of this, dermatology visits are considered medical evaluations , not preventive care, even when the visit feels routine or is for a full-body skin check.

What Codes Do Dermatologists Use?

Since dermatologists aren’t considered primary care providers, we don’t use the same preventive billing codes. Instead, we use what's known as Evaluation and Management (E/M) codes —these are codes like 99203 or 99213.

These codes reflect:

The reason for your visit
What is evaluated or treated
Any discussion, diagnosis, or follow-up planning

Even if you’re coming in for a “just-in-case” skin check, the visit still involves a detailed exam and medical decision-making. That’s why it’s billed using E/M codes, not preventive ones.

Why We Can’t Change the Code to Preventive

Sometimes patients ask if we can bill the visit as preventive to reduce their out-of-pocket cost. The answer is no—and here's why.

If we were to bill your visit using a preventive care code that doesn't apply to specialists, your insurance will likely deny the claim entirely . When that happens, the insurer won’t pay anything, and you could be responsible for the full cost of the visit out-of-pocket .

We’re required to use billing codes that accurately reflect the type of visit and the provider who is delivering care. Using the wrong code—even with good intentions—can lead to denied claims and unexpected bills.

“But My Insurance Said You Can Just Change the Code…”

Many patients tell us that they called their insurance company and were told something like, “Just ask your doctor to change the code to preventive and it will be covered.”

This is a common misunderstanding. Insurance representatives often aren’t trained in the specifics of medical coding. What they’re really saying is that codes can be changed in general—but they don’t usually understand that there are strict rules about which codes can be used by which providers.

Yes, codes are editable in a billing system when errors occur—but if we change a dermatology visit to a preventive care code, it will still be rejected because it doesn’t fit the guidelines for specialists. That leaves you responsible for the entire bill. So while the rep isn’t technically wrong that codes can be changed, they are often unaware of the consequences of doing so incorrectly.

What If I’m Just Getting a Skin Check?

We understand that it may feel like a preventive visit when you’re just getting a skin check. But because we are performing a medical evaluation—often looking for and addressing early signs of conditions like skin cancer—your insurance considers it a medical visit.

That’s why it’s not coded or billed the same way as an annual physical with your PCP.
